InForm Health & Exercise is a team of highly qualified Accredited Exercise Physiologists with a wide range of clinical experience and scope.  We specialise in individually tailored exercise programs and approaches for individuals with health conditions, complex needs, pre- and post-operative care as well as injury management. 

We are one of few allied health clinics which offer specialised breast cancer programs and mental health programs and we are so proud to be bringing such valuable resources to our wider community.

Ismini Tsakiridis
Director / Exercise Physiologist

I am an Accredited Exercise Physiologist with Exercise and Sports Science Australia.  I have completed a Bachelor of Exercise and Sports Science as well as a Masters in Clinical Exercise Physiology.

I am extremely passionate about providing safe and effective exercise programs to all my clients in helping them to achieve their set goals.  

My varying industry experience in both the public and private health sector assists me in providing a quality and in-depth health care service for all.

After completing my post-graduate studies I was awarded the honour of being names the recipient of the Exercise & Sports Science Australia Graduate of the Year in 2013.

Rochelle Gannon
Exercise Physiologist

I have over 14 years experience working in rehabilitation as an Exercise Physiologist. I have completed a Masters degree in Exercise Rehabilitation and have a Bachelor of Applied Science in Human Movement.

I have extensive industry experience in treating oncology, chronic pain, cardio-respiratory, neurological and orthopaedic conditions. My particular passion is striving to get better outcomes for breast cancer patients entering the survivorship phase of their cancer treatment.

  • 3 years experience assessing, delivering education and facilitating the exercise component of the Epworth Enhance Breat Cancer Rehabilitation program.

Kate Zoch
Exercise Physiologist

I am an Accredited Exercise Physiologist with Exercise & Sports Science Australia.  I have completed my Masters in Clinical Exercise Physiology and a Bachelor of Exercise & Sports Science at the Australian Catholic University. 

I have experience working with a variety of neurological, cardiac, orthopaedic and pulmonary conditions and have a particular interest in women’s health and geriatric rehabilitation.

Ultimately, I strive to provide quality and meaningful treatments to all of my clients.

Emma Young
Exercise Physiologist
I am an Accredited Exercise Physiologist with Exercise and Sports Science Australia. I have completed my Masters of Clinical Exercise Physiology and a Bachelor of Exercise Science/ Bachelor of Business Administration at Australian Catholic University.

I am passionate about helping clients accomplish their goals in a safe and supportive environment through the use of evidence based practice. I truly enjoy getting to know my clients so that they can achieve the best results possible.

  • Neurological conditions in particular those with spinal cord injuries and cerebral palsy
  • Cardiac conditions including heart failure and hypertension
  • Musculoskeletal conditions including complex fractures, amputations and lower back pain

I completed a Bachelor of Exercise and Sport Science and a Master of Clinical Exercise Physiology from Deakin University, before gaining accreditation as an Exercise Physiologist in 2016.

In my short career I have worked in both public and private hospitals, as well as in the private practice setting, where I have delivered tailored exercise services to clients with a wide range of health conditions.  In particular, I am passionate about the use of exercise as medicine for those who suffer mental health, cardio-respiratory, cancer and neurological conditions.

Phoebe Jones
Exercise Physiologist

I am an Accredited Exercise Physiologist with Exercise and Sports Science Australia upon completing my Masters of Clinical Exercise Physiology at Deakin University. I have also completed a Bachelor of Exercise and Sports Science.
I have a strong passion for delivering best-practice, evidence based exercise to improve the health and wellbeing of individuals with acute, sub-acute or chronic medical conditions. I have a particular interest in exercise and oncology and in delivering safe and effective individualised programs to assist patients in cancer prevention, recovery and survival.
